Home health care is a strong fit when a patient needs skilled medical care or therapy but prefers to recover or manage their condition at home rather than in a facility. If your family member has recently been discharged from a hospital, is managing a chronic illness, or needs help with daily activities, we can help you assess whether our services are appropriate.

Yes. Medicare covers skilled home health services — including nursing, physical therapy, occupational therapy, and speech therapy — when a physician certifies that the patient is homebound and requires skilled care. Medicaid coverage varies by state. We also accept most private insurance plans. Our care coordinators can help verify your coverage before services begin.

In most cases, we can begin a care assessment within 24–48 hours of your initial contact. For urgent situations, we work to expedite the process. Our care coordinator will walk you through the intake process, verify insurance, and schedule your first visit as quickly as possible.

We make every effort to assign consistent caregivers to each patient. Continuity matters — it builds trust, reduces anxiety, and allows caregivers to notice subtle changes in a patient's condition. When a regular caregiver is unavailable, we ensure a qualified substitute is briefed on the patient's care plan and preferences.

We maintain a 24/7 on-call line for all active patients. If you or your family member has a concern between visits, a care coordinator is available to advise, triage, and dispatch a nurse if needed. You are never left without support.

Every patient receives an individualized care plan developed by our clinical team in collaboration with the patient's physician and family. The plan is reviewed regularly and updated as the patient's condition changes. We communicate proactively with the patient's broader care team to ensure continuity.
